| Membership Fee Details |
As you are applying for membership to this Club we need your acknowledgement that you are aware of the club policies and that you accept the Clubs Code of Conduct and conditions of acceptance. You should be aware of the Clubs financial arrangements.
Membership runs from 1st July to 30th June and the fees are as follows: Juniors - 7yrs to 11yrs £130 Youth - 12yrs to 18yrs £144 Seniors - 19yrs plus £160
Players joining mid season will pay £40 then pro-rata at the appropriate rate. |
| There are 3 alternate methods of payment: |

Annual - Payment in full (if a renewing Member chooses this option then they will receive a £15 discount). |

Quarterly - Initial payment of £40. Juniors - 3 x £30 on 1st October, 1st December, 1st February. Youth - 3 x £35 on 1st October, 1st December, 1st February. Seniors - 3 x £40 on 1st October, 1st December, 1st February. |

Monthly - Juniors - £11 per month standing order payable on the 1st of the month (September to May) - starting the month after joining. These can be paid as cash/cheque or by standing order.
Youth - £12 per month standing order payable on the 1st of the month (September to May) - starting the month after joining. These can be paid as cash/cheque or by standing order.
Seniors - £13.50 per month standing order payable on the 1st of the month (September to May) - starting the month after joining. These can be paid as cash/cheque or by standing order.

Cheques should be made payable to MK Wanderers FC. and a receipt will be issued. |

No player’s registration application will be accepted without the required membership fee being paid at the same time. |

All fines imposed by the Leagues due to infringement of the laws of the game are to be paid by the player/parent themselves. In addition the Club will fine each player £2 for each booking received and £5 for a sending-off offence. Should a player be charged with using foul language then the Club will impose a fine of £5 in addition to the aforementioned. |

Details and limitations of the Clubs insurance policy are available from the Manager.
We try to constantly improve the advertising and awareness of the Club by increasing our presence in the local papers and also by promoting our own web site. We would therefore like your written permission to take photographs of younger players at matches and to use them for this purpose. Only names and ages will be used for this as the Club operates a data protection policy which ensures that only its’ own Coaches have access to the details on the Application form. Should you wish any particular photograph to be removed then this will be done immediately upon receipt of a written request by the Club Secretary.
All Club Coaches are F.A. qualified have a minimum first aid qualification, we need parental/guardian authority for treatment to be administered in the event of an injury during either training sessions or matches. We therefore need written permission for First Aid to be given to younger players by qualified first aiders or for medical treatment to be given by Ambulance or Hospital staff, including urgent surgery or blood transfusions, in the event of an emergency and when the Club is unable to contact you. Any injury to a young player, no matter how minor, will be reported to the parent or guardian as soon as practicable.
